School nurses and health visitors interviewed mothers at home about the cohort child’s health, schooling and employment, and their own and their husband’s health, and their home and family circumstances, and health visitors and school nurses also assessed the state of the dwelling. Teachers completed a postal questionnaire about the cohort child’s absences, attitude to school work, behaviour, habits and preparations for school leaving. Teachers also administered at school the questions to the cohort child about preparation for school leaving (printed at the front of the cognitive test booklet) and the cognitive tests. Head-teachers completed the postal questionnaire about the school and its amenities and services for career guidance, and about the school’s socioeconomic profile. School doctors carried out the medical examination. Hospitals to which cohort member children had been admitted were asked to complete a postal questionnaire about health, length of admission, visiting arrangements, diagnosis, treatment and discharge.
